Per Local Rule 9090-2(C)(4) a Motion to Maintain Prepetition Bank Accounts must include the following:
(a) a schedule listing each prepetition bank account that the debtor seeks to maintain post-petition; (b) the reason for seeking such authority; (c) the amount on deposit in each account as of the petition date; (d) whether the depository is an authorized depository under 11 U.S.C. § 345(b); and (e) a representation that the debtor has consulted with the Office of the United States Trustee about the continued maintenance of prepetition bank accounts and a representation about whether the United States Trustee has consented to the proposed maintenance of use of the accounts.
If the debtor is unable to provide the information in sections (a)-(e), the motion must explain why it is unavailable and must estimate when the debtor will supplement its motion with the information.
Step-by-Step Procedure:

Select [Bankruptcy>Motions/Applications]

Enter case number and click Next

Select Maintain Prepetition Bank Accounts from pick list and click Next

Select the Party you represent from the party pick list or if not listed then Add/create new party
-
Click Next
-
Click Browse button and navigate to locate the appropriate PDF
-
Select PDF to be uploaded into CM/ECF

Click Open button and filename will appear in the browse window
NOTE: The proposed order must be added as an attachment to the notice of motion and motion. -
Select Yes radio button for Attachment to Document and click Next

Select one or more attachments by clicking the Browse button and navigate to locate the appropriate PDF

Select PDF attachment to be uploaded into CM/ECF

Click Open button - filename of attachment will appear in the browse window

Complete the Category and/or Description
-
Click Add to List (attachment file name will appear in window)

Click Next
-
In text box, type any additional text to match PDF title (this is optional) and click Next

Click Next
-
Insert hearing date, time and select hearing location

Click Next
-
Screen with case information will appear - click Next
-
Verify the accuracy of the docket text - if applicable, you may make a selection from the prefix drop-down box - click Next

The Final Text page will display. This is your final opportunity to modify the entry before submitting the filing.

Click Next to complete filing process
The Notice of Electronic Filing will display. It is the verification that the filing has been sent electronically to the court.
Copies of this notice and the document filed are emailed to all participants who receive electronic notification in the case. You have a period of 15 days for a one time free look at the document filed.
